Client Services that we offer

Our case manager guides each client through the health care system and provides counseling and information about other community resources.

We have assisted over 9,000 women and men in the past 11 years who have qualified for our program with breast screening, diagnosis and treatment cost of breast cancer.

Client Qualification and Treatment Funding Guidelines

• Clients are a resident in Charlotte, Collier, Hendry, Glades and Lee Counties with a Social Security number.

• Clients who require breast care and have Medicaid “Share of Cost” that does not cover the cost of the procedure.

• Mammograms performed by local radiologists are funded at $65.

• Biopsies and other breast care procedures are funded only when recommended by a radiologist.

• When a negative result from a needle biopsy is reported, no further procedures are funded by Partners.

• Diagnosed clients requiring a lumpectomy or mastectomy are referred to Lee Health. When no funding is available through Lee Health, the client is referred to one of our breast surgeons (Partner in Health) and the cost is covered by Partners for Breast Cancer Care, Inc.

• Only one surgical follow-up consultation is funded.

• Clients requesting financial assistance are at or below 250% of the Federal Poverty Guidelines.

$2,501 a month or less for a single person

$3,383 a month or less for a family of two

$4,254 a month or less for a family of three

$5,125 a month or less for a family of four

$5,996 a month or less for a family of five
