Our Program....

Client Services

 

A case manager guides each client through the health care system and provides counseling and information about other community resources.

 

Through May of 2007, we have assisted over 6000 women and men who have qualified for our program with the diagnosis and treatment cost of breast cancer.

 

All of our funds are raised locally through grants and donations. We receive no federal, state, county or city funding.  We are a United Way agency and they state that 90% of funds raised are utilized for client services and treatment.  We are audited yearly by Stroemer, Tuscan & Company, PA.

 

Lee County Breast Coalition

 

Mission Statement -   To improve access to breast health care and treatment for low income and uninsured residents of Lee County.

 

In 2004 the Coalition was formed in Lee County consisting of everyone that assists women with breast cancer from the hospitals, diagnostic facilities and any available programs related to breast cancer.  Partners is now actively referring women from our program to the Florida Breast and Cervical Cancer Early Detection Program at Family Health Centers.  Please contact us for more information regarding this program.  The coalition also was able to educate everyone on what is available in our County for women and men.

 

 

 

Eligibility

  • Must have lived in Lee County continuously for the past 12 months
  • Must have no health insurance of any kind
  • Must have a prescription from a doctor (i.e. for a mammogram)
  • Must have a valid social security card
  • Must have Lee County ID with current address to prove residency
  • Must meet the 2008 financial guidelines of 200% of the poverty level
    • $1,733 a month or less for a single person
    • $2,333 a month or less for a family of two
    • $2,933 a month or less for a family of three
    • $3,533 a month or less for a family of four
    • $4,133 a month or less for a family of five
    • $4,733 a month or less for a family of six

Our program could not and would not exist without the support of our Partners in Health.  They are a very vital part of our organization and are devoted to helping men and women in Lee County who are in need of services.
